Face Procedures

Facelift (Rhytidectomy)

What does the procedure entail?

A facelift is an extremely common procedure that is able to provide a more youthful appearance by removing excess skin and fat on the face and neck. The procedure will typically take around 2-3 hours to complete, and you will wear a dressing over the cheek and neck area for the first 24 hours.

What will my recovery be like?

Fortunately, minimal pain is associated with a facelift. The average recovery time is 10-14 days, and most patients need two weeks before returning to work. Sutures will be removed within ten days, but avoid working out or raising your blood pressure for a full three weeks after.

Mid Facelift

What does the procedure entail?

A mid facelift is able to address the changes in your jowl and neck area, along with the lower eyelid and cheek. This procedure elevates the soft tissue from the mid-cheek back to the upper-cheek and lower eyelid area. Mid facelifts take about two hours, and you will wear a dressing around the cheeks and neck for the initial 24 hours.

What will my recovery be like?

Your sutures will be removed within ten days, and recovery will take between two to six weeks. You will be able to start working out again after three weeks, and can shower and wash your hair after 48 hours. Though some swelling and/or bruising is common, little to no pain is associated with a mid facelift.

Mini Facelift

What does the procedure entail?

A mini facelift is perfect for those that want a little lift, but do not want a full facelift. Traditionally, a mini facelift lifts the jowl area and leaves the patient with a more youthful appearance. The procedure is less invasive than a facelift or mid facelift, and it involves a small incision in front of the ear.

What will my recovery be like?

The recovery time for a mini facelift is 7-10 days, and does not involve any sutures or dressings. You can go back to work within two weeks, but should not workout until three weeks after the procedure. Minimal pain is involved with a mini facelift, and there is less bruising or swelling than a full facelift.

Endoscopic Brow Lift

What does the procedure entail?

An endoscopic brow lift is perfect for those that need a lift around their eyebrows, upper eyelids, and/or forehead. This procedure involves many small incisions near the scalp with an endoscope, all of which will be completely covered with your hair.

What will my recovery be like?

After the endoscopic brow lift is completed, the treated area will be covered with padding and a bandage to prevent any infections, swelling, or bleeding. The dressing will be there for the first couple of days, and the overall recovery time is ten to fourteen days.

Eye Surgery (Blepharoplasty)

What does the procedure entail?

A blepharoplasty lifts the eyelids to achieve younger looking and brighter eyes. Our surgeon will be able to trim sagging skin/muscle, and remove any excess fat and tissue around the eyelid. Once this is complete, our surgeon will use small stitches to complete the procedure.

What will my recovery be like?

The average recovery time for a blepharoplasty is seven days, but you will want to avoid raising blood pressure for three weeks, and prevent too much sun exposure. You may also experience swelling for up to ten days after the procedure.

Ear Surgery (Otoplasty)

What does the procedure entail?

For those with protruding or asymmetric ears, otoplasty may be the perfect option. Our surgeon is able to reshape patients’ ears with minimal surgery, depending on the exact problem they will be fixing. This procedure is done with general or local anesthesia with sedation.

What will my recovery be like?

Ear surgery is relatively painless, and the average recovery time is only seven to ten days. You may experience some degree of discomfort, swelling, and redness, but this should subside within the recovery period. You should be able to return to work within a week, but do not work out before three weeks after the procedure.

Neck Lift

What does the procedure entail?

As patients get older, the skin on their neck begins to sag. Fortunately, with a neck lift, this skin can be tightened, leading to a more youthful appearance. Our surgeon will tighten loosened muscles after performing a liposuction procedure on your neck. Depending on the patient’s needs, the exact procedure will vary.

What will my recovery be like?

For three days after a neck lift, you will have to wear a compression bandage for three days to make sure the healing process is successful. The recovery time is only seven days, on average, and you should be able to return to normal activities after this time.

Chin Augmentation

What does the procedure entail?

Patients choose chin implants for a variety of reasons, such as to improve facial proportion. Our surgeon will consult with you to determine the perfect fit for your needs, so you can feel confident in the results. The implants at CCLS are made of a synthetic material that is similar to your body’s natural facial tissue. The actual procedure will involve our surgeon making an incision in the chin, where he will then gently stretch the tissue to fit the implant in the chin.

What will my recovery be like?

Recovery time is seven days, depending on the exact procedure. Many patients find they can get back to their normal activities within the week, but your chin will be a tad sore and swollen. You may have a small bandage or dressing to make sure the implant stays in the right position in case there is swelling. A chin augmentation is a relatively pain-free procedure, but some patients do take oral pain medication for a couple of days after.

Nose Surgery (Rhinoplasty)

What does the procedure entail?

Rhinoplasty is extremely common, and Dr. Rubach has performed over 1,000 of these surgeries. Patients turn to rhinoplasty to improve the aesthetic look of their nose, though nose surgery is also used for medical reasons as well. During your consultation, Dr. Rubach will determine if an open or closed approach will be appropriate, and he will be able to tell how extensive the surgery will need to be.

What will my recovery be like?

Typical recovery involves a splint on the outside of the nose and an intranasal splint for one week. If the procedure involves an external approach then an incision is made at the base of the nose. Intranasal rhinoplasty does not involve an external incision. The first week requires sleeping with the head elevated to decrease swelling. At one week after surgery, the splints and sutures are removed. Typically, swelling or bruising take 10 to 14 days to subside. I often tell my patients to expect 2 weeks recovery before the signs of surgery will subside.

What should you avoid after a rhinoplasty?

The initial recovery phase of rhinoplasty is one week. During this time, an external splint must be worn, and typically an intranasal splint is in place. Patients may lightly blow their nose and clean any dry blood in the nostrils that would impede breathing. It is highly advisable to sleep with the head slightly elevated at a 30 degree angle to reduce swelling. I ask patients to avoid strenuous exercise for three weeks following the procedure. Also, while the splint is in place, avoid showering the face. Patients may however body shower or wash their hair inn the sink if they keep the nasal dressing dry.

Are nose jobs painful?

There are many aspects to post-operative pain following rhinoplasty. It is not typical for my patients to experience significant pain following their procedure. This is due to several technical aspects to my procedural approach to Rhinoplasty. First, I do not use packing and instead use thin internal splints. Packing is very painful and uncomfortable. Second, I use a local anesthetic called Marcaine, to numb the surgical site that lasts for 8 to 10 hours following the procedure. This provides anesthesia to the surgical area during the immediate recovery phase where pain would be most acute. There is discomfort from swelling, however this can be reduced by sleeping with the head elevated at 30 degrees for the first week.

Can rhinoplasty improve my breathing?

Rhinoplasty is surgery of the nose to improve form and function. It is essential that each patient being evaluated for rhinoplasty be evaluated for function, meaning the ability to breath, as well as for form, which is the overall shape of the nose. Both are very interrelated. Change in shape can have very significant impact on breathing and can significantly reduce nasal airflow. Factors that affect breathing are deviation of the septum, turbinate size, and support of the lower nasal tip cartilages that affect the shape and opening of the nasal airway at the base of the nose. The septum is the bony and cartilaginous structure that divides the left and right side of the nasal passage. Deviation from trauma to one side or the other can create obstruction of nasal airflow. The turbinates are internal structures arising from the lateral nasal wall of the nasal passage that humidify air. These can swell due to allergies or other causes and can cause obstruction. Lastly, the lateral wall of the nasal tip requires support from the underlying cartilage. In some cases there is not enough support and the lateral nasal area collapses with inspiration causing nasal obstruction. All of these issues must be evaluated on the initial consultation for rhinoplasty as they can be addressed at the time of surgery to improve nasal airflow. Septoplasty, turbinate reduction or reconstruction of the lateral nasal wall at the time of rhinoplasty can significantly improve breathing.

How does a rhinoplasty change your face?

The nose is the central structure of the face and thus affects the balance of the surrounding facial features. Additionally, chin projection, or lack there of, creates balance and harmony across the face. It is very common to discuss nasal and chin structure in tandem to achieve the proper balance to the facial structure. Computer imaging is essential during the initial consultation to view the proposed changes from rhinoplasty It is not only an important communication tool for the surgeon and patient to view during the consultation process, but it also provides a guide for the surgeon intraoperatively to achieve the desired outcome.

Will rhinoplasty affect the way I sound when I speak?

Improvement of nasal airflow can be achieved with rhinoplasty if there are underlying structural problems that are present that inhibit nasal airflow. If these issues are identified before surgery and then addressed at the time of surgery, then airflow can be increased post operatively and breathing can improve. Changes in nasal airflow can slightly change the sound when speaking by changing the degree of nasal airflow which affects resonance. The change however should be minimal.

What age should you get a rhinoplasty?

It is important to avoid structural changes to the nose until after puberty and facial development has taken place. This typically has taken place before the age of 16 however there are significant variations in the age for onset of puberty and completion of facial growth and this needs to be taken into account when deciding when to proceed with rhinoplasty.

Laser Skin Resurfacing

What does the procedure entail?

Laser skin resurfacing is a non-invasive treatment that helps patients improve their skin tone, texture, clarity, and quality. The procedure is convenient and effective, along with safe for all skin types. During your consultation, our experts will determine the best laser to use, along with the amount of treatments that will be best for your skin and individual needs.

What will my recovery be like?

Depending on the exact treatment, your recovery will be extremely simple. You will have to apply a thick layer of aquaphor every day to the treated area for seven days, but this is a painless process. The treated area will be red for a short while, but you should be able to return to normal activities quickly. Avoid any sun exposure for two weeks, and no makeup can be worn for ten days after.

Body Procedures

Breast Augmentation/Lift

What does the procedure entail?

Whether you decide that a breast augmentation or a breast lift is the right option for you, it is important to always choose an experienced surgeon that you feel comfortable with. At CCLS, we will take the time to discuss all of your options, and show you before and after results. A breast augmentation involves either silicone or saline implants, and they will be inserted either beneath or outside of the muscle. The implants will be inserted either in the lower crease between the breast and ribs or around the areola. A breast lift is often paired with a breast augmentation, but a lift involves removing and tightening excess skin around the breast.

What will my recovery be like?

Every patient has a bit of a different recovery, particularly if a breast lift is paired with a breast augmentation. However, there are guidelines to aid in your recovery. You will be able to resume your normal diet immediately, and are encouraged to walk right after, though you will want to wait to do any brisk walking for two weeks and running for four weeks. Avoid raising your elbows above your shoulders for a week, and don’t lift more than five pounds in each hand for three weeks. Prior to three weeks after surgery, you will not be able to wear any underwire bras. You will have follow up appointments to remove sutures and check on your recovery.

How do I prepare for breast augmentation?

Once you have chosen to proceed with breast augmentation there are several things that you can do in preparation for your surgery. First in foremost you must be in good medical condition. This is determined during our consultation and our history and physical at which time we assess your overall health status pertinent to a breast augmentation. Assuming that you have no chronic medical conditions precluding Anesthesia necessary for a breast augmentation, you should also be free of any bacterial or viral infections prior to surgery.

Finally you should be at a stable weight. If you are in the process of losing weight you should maximize weight loss. If you have recently given birth and have been breast-feeding, you should be free from any lactation for at least three months prior to your breast augmentation.

Are breast implants permanent?

Breast implants are not permanent. Most implants have a warranty of approximately 10 years however it is not unusual for an implant to last 15 to 20 years. At some point depending on your age, you would require either removal or exchange of the implant for issues related to the implied self or the soft tissues around the implant.

Will breast implants need replacing?

Breast implants have a limited lifespan and therefore at some point you would require the implant replacement. Most implants have a warranty up to 10 years however it’s not unusual for implants to last 15 to 20 years. The main indication for implant replacement would be the eventual failure of the implant itself which is expected at some point in time.

How long after a breast augmentation can I work out?

After breast argumentation, I generally allow patients to begin walking briskly in 1 to 2 weeks, running in 3 to 4 weeks, and heavy lifting using more upper body in six weeks. Certain lower torso and leg exercises can be performed immediately as long as they do not engage the pectoralis muscle.

What are the best implants to get?

In my opinion that the most natural looking and natural feeling implant is the silicone gel implants. This comes in multiple shapes as well as multiple different consistencies with regards to their firmness. Being able to specify shape volume and consistency allows me to provide the most precise result to achieve your desired goals.

Saline implants are still available however their usage has declined significantly with the popularity of the silicone gel implants.

What type of implant is safest?

There is no implant that is significantly “safer” than another. All implants have similar risks with regards to implant rupture, capsular contracture and infection.

How do I know what size breast augmentation to get?

Determining the breast implant size, or the size of the augmentation, is always One of the most important questions a patient well ask. Sizing is determined by corroboration between myself and the patient. During the consultation the patient will express her desires and her goals of what she would like to achieve. This information will then be taking into consideration during the physical exam. At that time the patient’s chest size skin quality and the amount of natural breast tissue will also be measured and evaluated.

After examination I can discuss with the patient a range of implants which would be appropriate to provide hey beautiful long lasting men natural result. Finally, and exercise is performed utilizing actual breast implants which are worn by the patient using garments provided to get a strong visual feedback Allowing us to provide her desired goals.

Tummy Tuck (Abdominoplasty)

What does the procedure entail?

A tummy tuck is a great option for those that have lost a dramatic amount of weight or have had multiple children, as it is able to manage excess amounts of fat and skin. Oftentimes, those that have lost a lot of weight have excess skin that no amount of diet or exercise will be able to minimize. This procedure removes the skin and extra fat so that your abdominal muscles are more toned and firm. Ideal candidates are typically healthy and at a normal weight.

What will my recovery be like?

Most patients take one or two weeks to recover, but you will want to wait to exercise for a full three weeks (six weeks for any weight lifting). For three to four days after surgery, you will want to stick to small meals, and you will have to sleep on your back for one week. One of the best parts about a tummy tuck is that the results can last your whole life with regular diet and exercise.

Liposuction

What does the procedure entail?

Excess fat is often difficult to be rid of, even with regular diet and exercise. For those that are at a relatively healthy weight, but have pockets of fat that they cannot seem to get rid of, liposuction is a great option. In your consultation, you’ll be able to discuss the areas on your body you want to see improvement on, whether your arms, stomach, back, etc. You will be given anesthesia for the actual procedure, and our surgeon will remove the excess fat with a cannula. Once you have recovered fully, the area will be better contoured and sculpted.

What will my recovery be like?

Liposuction recovery depends on the extent of the procedure, and the area or areas we have targeted. You will be able to resume a normal diet immediately, along with walk and do stairs after the surgery. Plan on wearing comfortable clothes, as you will need to wear an elastic girdle for up to a week post-surgery. Results will show after 48 hours, but swelling and bruising may last for up to eight weeks. You’ll be able to resume working out after two weeks.

Mommy Makeover

How do I get my pre-baby body back?

Having a baby can be one of the most exciting and fulfilling events in a woman’s life. However pregnancy and breast-feeding can certainly take a toll on a woman’s physical appearance.

The most common changes that occur from pregnancies are to the breast and abdomen.  In addition to some women may have significant fluctuations in their weight. Breast changes include stretch marks, skin laxity, nipple enlargement, and volume depletion.

Similar changes can occur to the abdomen. Loose skin, stretch marks, and increase fat are all common changes. In addition, the muscles of the abdominal wall can become stretched out or weakened.

There are multiple procedures to address all of the issues that have been mentioned above. The optimum results are achieved when diet and exercise are used in combination with surgical and nonsurgical modalities to treat the changes which have occurred during the pregnancies.

What is included in a Mommy Makeover?

Mommy makeover is a term used to describe procedures performed on women after their childbearing years. This usually includes change to the breast as well as the abdominal wall. These procedures include such things as breast augmentation breast lift liposuction and abdominoplasty, otherwise known as a Tummy Tuck.

How do you prepare for a mommy makeover?

After the childbearing years women considering a mommy makeover should maximize their weight loss prior to any surgical procedure. This would allow for obtaining the optimal results possible. Certainly the patient should have completed breast-feeding and should know longer be lactating. In addition to any other underlying medical conditions should be addressed by their gynecologist or primary care physician in preparation for a surgical procedure. Finally, as with any surgical procedure, a patient should be free of any nicotine products including cigarette, nicotine patches, or vapor devices.

How long does it take to recover from a mommy makeover?

Depending on the procedures performed as instructed by her physician, recovery is generally in the 4 to 6 week period to return to full unrestricted activities and exercise.

Should I get a mommy makeover if I plan on having more children?

If a patient is considering having more children it would be unwise to have a mommy makeover. Additional pregnancies would have the same effects on the breast and abdomen as the initial pregnancy did. Therefore you may require further revisionary procedures. While there is no contraindication to becoming pregnant after a mommy makeover it would be much more practical to wait until after the childbearing years have been completed.

Male Breast Reduction

What is a male breast reduction?

Male breast reduction is a procedure designed to help men with a very common problem known as gynecomastia or male breast enlargement. I am excited to see that more and more man understand at this condition can be treated and are seeking help for it. Male breast reduction surgery is a broad term which includes multiple technics which can be used to achieve a more masculine chest shape. These procedures address A multitude of issues which could include enlargement of glandular tissue, excess fat, excess skin, or feminization of the nipple areolar complex. These techniques can include cool sculpting, Liposuction or surgical excision.

These strategies are individualized to match each patient’s preoperative condition to yield a customized result.

Non-Surgical Procedures

Which non-surgical face lift is the best?

Nonsurgical facelift is truly a misnomer as utilizing Botox and fillers does not accomplish a true facelift. Facial aging nearly always includes a combination of volume depletion and laxity of soft tissues. Combining lifting and repositioning along with appropriate volumization achieves natural and optimal results. The nonsurgical facelift is described as utilizing fillers to achieve a facelift. This simply is not the case. Fillers can result in subtle lifting of soft tissues however, utilization of fillers alone to try and lift soft tissues in a more significant way very often results in the overfilled and unnatural appearance. I do use hyaluronic acid fillers to treat the aging face. There are a multitude of HA products, all with different properties, that are utilized at different levels of the skin and underlying soft tissues to diminish visible signs of facial aging. Significant improvement can be achieved with fillers alone however with significant soft tissue laxity, face lifting is also necessary to achieve optimal results.

What is the best fat removal procedure?

There are several non surgical options for fat removal with todays leading edge technologies. The first is Coolsculpting which utilizes a handpiece that is applied to the area to be contoured and freezes the underlying fat cells. Once the fat cells are frozen, they die and are absorbed. This noticeably reduces the contour of the treated area. Typically, the area is reduced by 25% with a single treatment. It is possible to have multiple treatments in an area. Another option is Kybella, which is an injectable treatment to reduce fat. The area to be treated is injected with small amount of Kybella. This does require multiple injections at the treatment site. Recovery generally takes one week and is primarily related to swelling. Kybella often requires multiple treatments. Choosing the ideal procedure is based on the area to be treated and the specifics of the patients physical exam. It is essential to have a consultation with a highly experienced provider so that the correct recommendation can be made.

How do I get rid of fine lines and wrinkles?

Skin lines and wrinkles are caused by loss of skin structure, meaning reduction of collagen and elastin, and by contracture on the surface of the skin caused by muscle movement. Factors that affect development of wrinkles are intrinsic and extrinsic factors. Intrinsic factors are related to genetics which determine  the amount of collagen and structure in the skin. Fair skin types age more quickly than darker skin types. Extrinsic factors include sun exposure and sun damage, nutrition, smoking and others. The primary treatment is botox which relaxes the muscle and minimizes skin contracture and wrinkling. This typically lasts 3 to 4 months. Hyaluronic acid fillers are another treatment option for wrinkles and skin creases. The filler is injected into the wrinkle or fold and typically lasts for 10 to 12 months. Other therapies are directed to improving skin structure. These therapeutic options are topical therapies, light and laser treatments. The leading edge laser treatment is the Halo laser .This is a fractional laser that includes two wavelngths. The first is the erbium laser, 2940nm wavelength, that treats the surface epidermis. The second is the non ablative wavelength of 1470nm that treats the deeper dermis. The result is improved collagen, tightening of pores, diminished fine lines and wrinkles. This is done with topical anesthesia and requires two to four days for recovery.

What is the best procedure for sun damaged skin?

Sun exposure damages skin by injuring collagen and elastin fibers, creating inflammation and redness, and irregular pigmentation. BBL or Broad Band Light treatment is a non invasive treatment to reduce redness and irregular pigmentation. It involves a brief flash of light on the surface of the skin. The treatment is quick, relatively painless and requires no downtime. BBL essentially improves the look of the skin by creating a more even tone to the skin. Increasing the amount of collagen in the skin is accomplished with laser treatments. A leading edge minimally invasive laser treatment is Halo. This is a unique laser that incorporates two laser wavelengths. The first is the erbium laser, 2940nm wavelength, that treats the surface epidermis. The second is the non ablative wavelength of 1470nm that treats the deeper dermis. The result is improved collagen, tightening of pores, diminished fine lines and wrinkles. This is done with topical anesthesia and requires two to four days for recovery.

How long do lip fillers last?

Fillers come in many different types. The most common is hyaluronic acid which is based on a natural sugar molecule found in skin and joints. The sugar molecule is joined together in long chains that give it a gel like consistency. This is ideal as a filler as it is natural, feels natural, and is reversible if needed. There are several different HA fillers that each have different characteristics that provide unique properties that are advantageous for different uses. These properties create varying lengths of duration of the product. One property is G prime which represents cohesiveness of the filler. This in short is the property of resisting deformation.  Think of this as the filler resists the ability to compress and change the shape. This is very important is some fillers as we do not want the filler to change shape if pressure is applied, such as lying down at night with your face on a pillow. If we place filler in the cheeks to restore facial volume, we want the shape to be stable despite pressure on the tissues. Another property is viscosity. Fillers placed more superficial require less viscous properties to avoid contour irregularity whereas fillers place deep in the subcutaneous tissue should be more viscous to more closely resemble fat.

Getting back to how long fillers last. Products used to treat more superficial lines will have properties that make it last 6 to 10 months. Products that are injected into the deeper layers of the skin can last 10 to 12 months while products injected into the deep subcutaneous tissues to add volume can last 18 to 24 months.

Can chemical peels reverse sun damage? 

Chemical Peels can improve sun damage when done at a medium depth level. Usually they are done in a series to minimize downtime. For the greatest resolve for sun damage would be a laser treatment. The level of treatment would depend on the severity of the sun damage.

What is the right age for a facial?

Facials can start as early as the teen years to keep acne under control and throughout the rest of the years for anti-aging and skin health benefits. We offer a wide range of facials to accommodate specific patient needs.

How do I reduce my under-eye wrinkles?

Lower eyelid wrinkles occur in the lower eyelid skin and are due to loss of skin elasticity and collagen. There are several ways to address this issue however it is essential to make the correct diagnosis so that the proper treatment can be instituted. If the problem is limited to skin laxity, then lower eyelid resurfacing can be done with a laser. The laser is used to precisely remove the outer layer of skin and then new skin along with a significant increase in collagen occurs as the skin heals. This results in new, healthy revitalized skin. If there is significant skin laxity along with bulging of the lower eyelid fat pads, then a lower eyelid blepharoplasty may be the treatment of choice. This involves an incision just below the eyelash margin and the excess fat is removed or used to fill the hollow in the tear trough area and then the lax skin muscle complex is tightened and the excess is removed. Healing for each procedure initially requires one week.
